Here's what you need to know and do.

To serve you better, we will send by email or text a very simple, short, and secure survey to submit this information:

  • Names and dates of birth of all family members (whether or not they need coverage).
  • Your home zip code and contact information.
  • First and last names of your doctors. (Be sure to check the spelling of names so we don’t make a mistake.)
  • Name, dosage, and frequency of all medications (you can ask your pharmacist or doctor for this).
  • When your health insurance ends or ended, and when you want coverage to start.
  • For Under Age-65 health insurance plans only, a rough Household Adjusted Gross (PreTax) Income estimate for the calendar year. This is so that we can estimate any subsidy of your premiums or out-of-pocket medical expenses. Include all sources of income from all family members that are on your tax return for the year you need coverage.
